European studies conclude that 80% of healthcare spending is concentrated in the last six months of the patient's life. Heart failure is the leading cause of hospitalisation in people over the age of 65 and carries a high cost to healthcare systems and loss in quality of life.
The challenge
Three public procurers from Sweden, Italy and Spain will jointly purchase the design of a technology-enabled all-in-one service, flexible enough to adapt to a large variety of healthcare systems and enable the full deployment of the TIQUE's model.
The model will be tailored to each participant procurer, complementing existing public resources with a public-private partnership. Its solutions will include an interactive digital platform where clinicians and health and social care professionals can access their own case management platform (integrated with existing legacy systems). Patients and carers can log into their own self-management system.
The main target improvements are:
- 33 % increase of patient life expectancy;
- 40 % reduction of acute hospital admission;
- 20 % reduction of drug-related health problems;
- 20 % increase of home care attention; and
- 10 % increase of end-of-life care at home.

Hospital Sant Pau in Barcelona (Spain), Region Västerbotten (Sweden) and Azienda Sanitaria Locale Avellino (Italy).
